• Arthritis Foundation The mission of the Arthritis Foundation is to support research to find the cure for and prevention of arthritis and to improve the quality of life for those affected by arthritis. The Arthritis Foundation currently provides more than $20 million in grants to more than 300 researchers to help find a cure, prevention or better treatment for arthritis.

• Black Women's Health Imperative, the new name of the National Black Women's Health Project, is a leading African American health education, research, advocacy and leadership development institution. Founded in 1983 by health activist Byllye Y. • Kaiser Family Foundation  A substantial and growing body of research indicates that race/ethnicity – independent of clinical and socioeconomic factors – continues to matter in the U.S. health care system. Racial/ethnic background continues to affect access to health care and the quality of care obtained. While the evidence varies for specific conditions and racial/ethnic minority groups, the data are sufficiently compelling to begin undertaking actions to systematically and aggressively eliminate disparities in needed medical care. As major decision-makers in health care, physicians have a key role to play when it comes to addressing disparities in medical care. As a first step in what ultimately must be a multifaceted effort, The Henry J. Kaiser Family Foundation and The Robert Wood Johnson Foundation are undertaking an initiative to raise physician awareness about disparities in medical care, beginning with cardiac care.

• Robert Wood Johnson Foundation - Health Care Philanthropy   Racial and Ethnic Disparities link on left is to the latest information on the Foundation's activities in Reducing Racial and Ethnic Disparities in Chronic Care.

• The Robert Wood Johnson Foundation Diabetes Initiative addresses community and individual challenges to managing diabetes through two programs: Advancing Diabetes Self Management and Building Community Supports for Diabetes Care. Because self management is a critical element in all chronic care, the Advancing Diabetes Self Management program supports diabetes patient-centered educational programs. Likewise, community-based programs and linkages to effective community resources are recognized as important components of diabetes self management. The Building Community Supports for Diabetes Care program funds innovative community approaches to reaching high-risk populations. The Diabetes Initiative National Program Office (NPO) has been in existance since May 2002 and is housed in the Division of Health Behavior Research at the Washington University School of Medicine in St. Louis, Missouri. The NPO, funded by The Robert Wood Johnson Foundation, serves as the coordinating center for the two diabetes programs and provides training, technical assistance and consultation to the 14 grantees of these two programs.
